Diffuse Axonal Injury

  • shearing injury
  • deep white matter injury
  • deep axonal injury
  • Found at corticomedullary junction, corpus callosum, internal capsule, deep gray matter & brain stem region
  • CT : initially maybe normal. The appearance varies overtime depending on the amount of bleeding & the axtend of surrounding edema
  • MR : T1 often normal; T2 or FLAIR increased signal
DAI Pathophysiology
  • shear-strain injury.
  • different consistencies
  • strains at their junction
  • results in axonal injury and rupture.
  • the white-gray matter junction
  • deep centroaxial white matter
